Online surveys - a lesson in what not to do

Jared Spool recounts his recent experience of completing a survey for United Airlines, revealing 19 things not to do.

It's an entertaining way to learn a bit more about how to create a web form to collect user opinions, and some (hopefully fairly obvious) pitfalls to avoid.
